How to Request Services

Complete the Service Request Form Complete the Service Request Form To complete and submit a service request form log into the Service Request and Tracking System. This system allows you to track all your service request forms and calendar dates. We advise you to check your status frequently on the Service Request and Tracking System regarding service request and calendar date approval.
Request Calendar Dates Request Calendar Dates Once your service request form is approved by the appropriate lab lead and instrument lead you can schedule pending dates for your work via the Service Request and Tracking System. The appropriate instrument lead will be notified of your preferred dates and either approve or decline them. If your dates are declined the instrument lead will work with you to determine alternate dates.

The lab lead or CAES Safety Officer will contact the researcher regarding necessary information to complete this process.
Resubmit
Ship samples and conduct work Ship samples and conduct work All radiological samples must be shipped to and from CAES per Department of Transportation (DOT) and Idaho State University (ISU) requirements. Work with the CAES Safety Officer Kristi Moser-McIntire to accurately ship your radiological samples.

You can indicate the ship date and tracking number for your samples in the Service Request and Tracking System.

Once your sample is received you may begin work in CAES.

If CAES is shipping the item to you we will indicate the ship date and tracking number for your samples in the Service Request and Tracking System.

If your Service Request is complete, indicate it on the Service Request and Tracking System.
